Please keep in mind before submitting a form:

Maximum of 1 design per person. If you choose more than 1 design, I will not consider your inquiry.
If you are coming with a group/friend, each person will need to fill out their own form and come at their allotted time frame
Arm and leg placements only
$50 non-refundable deposit will go towards the tattoo and paid through Venmo or Zelle. Any cancellations and late arrivals result in forfeiting the deposit.
If I do not receive a response from you within 48 hours of my initial response to your inquiry, I will offer that time slot to the next inquiry

By clicking the button below, I verify that I am deposit ready and will be able to secure my deposit as soon as a time frame for me is available. I understand that this is a first come, first serve based flash day and I am not guaranteed a time slot that works for me. Under the circumstances that I need to cancel my appointment or if I arrive late, the non-refundable deposit is forfeited. As this is also a discounted tattoo, I understand that while there will be maximum efforts towards a perfect tattoo, there is still a chance of it being flawed.
